The Required: Men’s TRT Initial Panel provides a targeted assessment of key hormones and safety markers necessary for initiating testosterone therapy. Testosterone replacement requires careful monitoring to ensure therapy is appropriate, effective, and safe over time.

This panel evaluates testosterone levels alongside pituitary signaling, thyroid function, and prostate related markers to provide clinical context for treatment decisions. It is required within six months prior to starting testosterone therapy (including testosterone injections, testosterone cream, hCG, and enclomiphene) to evaluate eligibility.

What this panel evaluates

  • Comprehensive metabolic panel (CMP): Includes fasting glucose and markers of liver, kidney, and electrolyte balance
  • CBC with differential: Offers context on overall health and inflammatory or physiologic stress
  • Total testosterone: Measures overall testosterone levels
  • Free testosterone: Assesses biologically active testosterone
  • Estradiol: Evaluates estrogen balance and conversion, which can influence symptoms and treatment response
  • Luteinizing hormone (LH) and follicle stimulating hormone (FSH): Assess pituitary signaling to the testes
  • Thyroid function: TSH and free T4 to assess metabolic regulation
  • Prolactin: Assesses for hormonal influences that may suppress testosterone
  • Total + Free prostate specific antigen (PSA): Screens prostate health for men
  • Sex Hormone-Binding Globulin (SHBG): Assesses biologically active hormone levels
  • Adrenal and stress hormones: Urinary cortisol (AM) to evaluate stress response patterns
  • Androgen support: DHEA-S as a marker of adrenal reserve and resilience
  • Ferritin: Reflects iron storage and acts as an acute phase reactant in inflammatory states

 

How it works
The Required: Men’s TRT Initial Panel is a blood test that requires clinician approval. After your initial screening, it can be scheduled through this portal and completed at a lab near you. Scheduling your lab test

For accurate assessment, this lab panel must be completed between 7:00 and 10:00 a.m., when testosterone levels are at their physiologic peak. Tests drawn outside this window cannot be considered valid for treatment qualification.
